Intent/Affidavit

  1. File Int/Aff for Washington Prevailing Wage Jobs
    1. For Blue Mountain Action Council
        1. Username is: A1PLUMBING
        2. Password is: PrevailingA1
      1. Once you are logged in, click on the link labeled “File an Intent to pay prevailing wage or an affidavit of wages paid”
      2. Click Accept to the disclaimer
      3. To file an “Intent” to pay prevailing wage
        1. Under the heading “File New Intent” Select “I am the prime contractor for the project”
        2. And then click “Create Intent”
        3. For Contract Type Select: “Limited Public Works(Less then $50,000)
        4. Under”Awarding Agency:“ Enter “Blue Mountain Action Council” and click “Search for Awarding Agency”
        5. Click Select when they pop up in the search field down below.
        6. Fill out the remainder of the form
          1. For BMAC as follows
            1. Awarding Agency Contact: Terry Keller
            2. Awarding Agency Contact Number: 509-529-4980
            3. Awarding Agency Contract Number: Specific to each project and requires info from BMAC
            4. Project Name: This can be found on the Purchase Order Sheet from BMAC
            5. Is apprentice utilization required? No
            6. Is OMWBE utilization required? No
            7. Is this also a federally-funded project? No
            8. Project Site Address: Can be found on the Purchase Order Sheet from BMAC
            9. Project Description: This can also be found on the Purchase Order Sheet from BMAC
            10. Add Notes: This first notes field can just generally be left blank.
          2. Click “Continue to Next Step”
            1. Workers' Compensation Account ID: Select “404,045-02” from the drop down
            2. How many owner/operators performing work on the project own 30% or more of the company? “0”
            3. Does your company intend to hire ANY subcontractors? Generally the correct answer is “No” but if for some reason we will have subcontractors then adjust accordingly.
            4. Will your company have employees perform work on this project? “Yes”
            5. Do you intend to use any apprentices? (Apprentices are considered employees.) “No”(No is the correct answer until we have state approved apprentices, enrolled in a state approved apprenticeship program)
            6. Enter your estimated contract amount, (including sales tax) Or check the box for time and materials. Unless this is a bid job with a known contract amount, just mark the little box that says “Time and Materials”.
            7. Your expected project start date? Enter the appropriate anticipated start date.
            8. In what county (or counties) will the work be performed? Enter the county
            9. In what city (or nearest city) will the work be performed? Separate multiple cities with a comma. Enter the City
            10. Add Notes: This second pages notes field can just generally be left blank.
          3. Click “Continue to Next Step”
            1. Under the “Journey Level Trade” For BMAC jobs that are in homes(houses) select “Residential Plumbers & Pipefitters.
            2. Click the “Add Wage rate for occupation” link to create line items for each worker that has a different payrate and fringe.(Usually this means 1 for each)
              1. Enter in the wage rate & the fringe, See the Spread sheet in the payroll binder for the info, or on the Y Drive Y:\! Personnel\Payroll Info and select the “Fringe Benefits Spreadsheet”.
            3. Add Notes: This third page notes field can just generally be left blank.
          4. Click “Continue to Next Step”
            1. When you do that, you should see a box pop up that states “In order for us to consider whether the residential rate is………………….
            2. Copy and paste the following Notes section of the form, per the instructions on the website
